By Alishahi Insurance, independent broker · CA License #4348151 · Updated October 2026 · En español

Short answer: Cal-COBRA lets you stay on a health plan from an employer with 2 to 19 employees for up to 36 months after losing coverage, at up to 110% of the premium. If you used 18 months of federal COBRA from a larger employer, Cal-COBRA can generally extend you to 36 months total. According to the DMHC, you must return the election form within 60 days and pay the first premium within 45 days.
